PS: The issue date of Household Registration should updated within one month from the day of application.

Remarks

1.

The applicant's English name should be as the same as it in his/her passport.

2.

If the applicant is prosecuted by the court but be acquitted later, he/she should present the written judgement.

3.

For people who apply for the certificate through online, bring the requirements when they want to pickt up the Police Record Certificate.

(1) Fill out the application form. Please provide accurate information and available phone numbers.

(2) After sending out the form. Remember to call at 886-3-3335107 to confirm between 8:00am-7:00pm Monday-Friday, working time.
(3) If the applicant cannot pick up the certificate personally, a notarized statement handed in to your agent is neccessary. If the applicant is minority, the legal respectives can act for the one.

4.

For those who want to pick up the certificate by themselves should bring the receipt with them.
