1.Required
(a)At-the-counter application:

Application form

For citizens, either of the two following documents:

ID card  ID card (original & copy)

 

*

Household Registration

-

For foreigners, either of the two following items:

 

*

Passport  Passport (original & copy)

 

*

Alien Resident Certificate (an orginal & copy one)

 

N.B. The issue date of Household Registration should be up-to-date (within one month from the day of application).

After the application is completed, please dial 886-3-3335107 for confirmation.

After three working days, please collect the certificate with the required documentation at our Foreign Affairs Service Center.

 

 

2.Remarks

(a)

The applicant's English name must be the same as his/her passport.

(b)

If the applicant is prosecuted by the court but be acquitted later, he/she should present the written judgment..

(c)

For people who apply for the certificate via online application, must bring the requirements when they wish to collect the Police Record Certificate..

 

* Fill out the application form. Please provide accurate information and available phone numbers.

 

*  After sending out the form. Remember to call 886-3-3335107 to confirm. Office hour is 8:00am-5:00pm Monday-Friday.

*  If the applicant cannot collect the certificate personally, a notarized statement handed in to your agent is necessary. If the applicant is minority, the legal representative can act for the one.

 

(d)

For those who wish to collect the certificate by themselves should bring the receipt with them..
